Terms of Service

By creating a VaultForms account, you agree to these Terms of Service and our Privacy Policy. If you are using VaultForms on behalf of an organization, you represent that you have authority to bind that organization to these terms.

VaultForms is available only to licensed healthcare providers operating in the state of Minnesota. By signing up, you represent that your agency holds valid licensure from the Minnesota Department of Human Services (DHS) or Minnesota Department of Health (MDH) for the services you provide.

Before collecting any Protected Health Information (PHI) through VaultForms, you must sign our Business Associate Agreement (BAA). Use of VaultForms to collect PHI without a signed BAA is prohibited and may result in immediate account suspension.

You agree to use VaultForms only for lawful purposes related to your licensed healthcare services. You may not: use VaultForms to collect data from individuals outside Minnesota; share account credentials with individuals not employed by your agency; attempt to reverse engineer or circumvent platform security; use VaultForms for purposes unrelated to your licensed healthcare services; or submit fraudulent or fabricated clinical data.

Free plan access is provided indefinitely at no charge with stated limits. Paid subscriptions are billed monthly or annually via Stripe. Subscriptions auto-renew unless cancelled. Refunds are not provided for partial months. Downgrade to the free plan is available at any time with no penalty. Price changes will be communicated 30 days in advance.

You own all data submitted through your agency's VaultForms account. VaultForms does not claim ownership of your agency data or your clients' health information. You are responsible for compliance with all applicable laws regarding the data you collect.

VaultForms targets 99.9% uptime. Scheduled maintenance will be communicated in advance. We are not liable for data loss or service interruption caused by circumstances outside our reasonable control.

You may cancel your account at any time. We may suspend or terminate accounts that violate these terms, fail agency verification, or engage in fraudulent activity. Upon termination, you may request a data export within 30 days.

VaultForms is a software tool. We are not responsible for clinical decisions made based on data collected through our platform. Our total liability for any claim is limited to the amount you paid us in the 12 months preceding the claim.

These Terms are governed by the laws of the State of Minnesota. Any disputes will be resolved in the courts of Hennepin County, Minnesota.
